ข้าพเจ้ายินยอมให้มหาวิทยาลัยเชียงใหม่ เก็บ รวบรวม และใช้ข้อมูลของข้าพเจ้า เพื่อประโยชน์ในการบริการรับวัคซีนโควิด โดยยินยอมให้มีการเปิดเผยข้อมูลเฉพาะหน่วยงานที่ได้รับหมอบหมายจากสำนักงานสาธารณสุขจังหวัดเชียงใหม่ให้เป็นผู้ประมวลผลและจัดทำระบบข้อมูลดังกล่าวเท่านั้น

        ข้าพเจ้าขอรับรองว่า ข้าพเจ้าขอรับรองว่า ข้อมูลที่ข้าพเจ้าให้เป็นข้อมูลส่วนบุคคลของข้าพเจ้า ซึ่งเป็นข้อมูลที่แท้จริง ถูกต้องและเป็นปัจจุบัน

Agreement and consent to data

        I consent to the Chiang Mai Provincial Health Office and Chiangmai university, to collect and use my data for recieve the Covid-19 vaccine. By allowing the disclosure of information, only the agencies assigned by the Chiang Mai Provincial Health Office are allowed to process and creating the data system only.

        I certify that The information that I provide is my personal information which is true information, accurate and current.
